Agent No Longer at Office

This form is used when an agent/salesperson is no longer working at the office listed through their membership. This form must be signed and submitted by the Designated REALTOR®/Broker of the office.

Agent Office Transfer

This form is used whenever changing your user/MLS information or when transferring from one office to another. This form must be signed by the Designated REALTOR®/Broker of the office to be considered valid.

Broker Office Add/Change Request

This form is used in order to make changes to existing firms (address, phone number, firm name, etc.) or to add offices/branches. This form must be signed and submitted by the Designated REALTOR®/Broker of the office.

Designated REALTOR® Appointment of Manager

This form is used whenever a Designated REALTOR®/Broker wishes to give office manager rights to an agent within the office. This form must be signed and submitted by the Designated REALTOR®/Broker of the office.

Membership Information Change Request

This form is used whenever you want to update your membership information such as phone number, address, and e-mail address.

Membership Termination Request

This form is used whenever you are terminating any form of membership with the California Desert Association of REALTORS®. Only services that have been checked will be terminated, so please make sure the form is filled out carefully and accurately. This form must be signed by the Designated REALTOR®/Broker of the office to be considered valid.
